Canine body workers are NOT licensed veterinarians; canine body work is NOT a substitute for veterinary care. If the canine displays signs of lameness or illness, body work may not take place until permission of the attending veterinarian has been granted.

Canine body workers may not make any adjustments/musculoskeletal manipulations.

Canine body workers may not prescribe medication of any type.

It is recommended that you consult with your veterinarian regarding complementary care for your canine.

This agreement affects your legal rights. Please read it carefully.

I, being the authorized agent or owner of this canine, have read and understand the information provided by this form and:

1. I understand that body work is NOT a substitute for veterinary care, and further acknowledge that the body worker does not diagnose illness, disease, or any other physical disorder.

2. The canine body worker does not prescribe or administer medical treatment.

3. It is my responsibility to consult with a veterinarian regarding complementary care for my canine prior to starting any bodywork, exercise or stretching protocol.

4. I waive any and all liability, claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained, or to any damage to property, including the canine, belonging to the agent or owner of the canine, whether caused by the negligence of the body worker, or otherwise, from the services provided in the session including but not limited to massage or other soft tissue modalities while on the premises where the session is being performed.

5. I waive any and all liability, claims, demands, actions and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained, or to any damage to property, including the canine, belonging to the agent or owner of the canine that occurs from performing any suggested exercise or stretch whether performed in the presence of the body worker or independently.

6. I understand that if I cancel the appointment less than 48 hours before it is scheduled to take place, I will be subject to a cancellation fee equal to your session fee. To avoid a cancellation fee, please provide cancellation notice at least 48 hours prior to your appointment.
